Depending on the participant’s condition and needs they may require the expertise and experience of a specialised worker. Capital Community Care is able to provide high intensity supports with our well trained, qualified and certified Registered Nurses and enrolled nurses to ensure you receive the optimum care you deserve. High intensity support is divided in to three levels which are depending on participant needs and staff qualifications. Capital community care capable to deliver support across Level 1, 2 and 3. With a full understanding of the care required for participants our experienced staff can provide various high intensity services for your loved ones which will improve their quality of life and comfort.

Types of High-Intensity care.

Capital Community Care Pty Ltd will ensure that each participant who receive NDIS funded supports in relation to enteral nutrition and management has the right to quality of care to maintain and support their health and wellbeing. Whenever possible the participant must be involved with decision making and exercising choice about care and management of their specific health needs.

We will ensure that any participant who requires support with complex bowel care receives an appropriate level of support that is relevant and proportionate to their needs. Any participant that receives support with complex bowel care is involved in assessment and development of their Complex Bowel Care Plan.

Any participant that receives support with urinary catheter management will be involved in assessment and development of their management plan. The Plan and relevant procedure will identify how risks, incidents and emergencies will be managed, including required actions and escalation to ensure participant wellbeing

Each person who receive NDIS funded supports in relation to Complex Wound Management have the right to quality of care to maintain and support their health and wellbeing. Whenever possible the participant must be involved with decision making and exercising choice about care and management of their specific health needs. All workers working with a participant requiring complex wound management have training, relating specifically to the participant’s needs that are affected by their wound management regime (for example, showering, toileting and mobility) and high intensity support skills descriptor for providing complex wound management, delivered by an appropriately qualified health practitioner or person that meets the high intensity support skills descriptor for complex wound management.

We will ensure that each participant requiring subcutaneous injections receives appropriate support relevant and proportionate to their individual needs and specific subcutaneous injections and medication administered.
