Depending on the participant’s condition and needs they may require the expertise and experience of a specialised worker. Capital Community Care is able to provide high intensity supports with our well trained, qualified and certified Registered Nurses and enrolled nurses to ensure you receive the optimum care you deserve. High intensity support is divided in to three levels which are depending on participant needs and staff qualifications. Capital community care capable to deliver support across Level 1, 2 and 3. With a full understanding of the care required for participants our experienced staff can provide various high intensity services for your loved ones which will improve their quality of life and comfort.
Types of High-Intensity care.
1. ENTERAL NUTRITIONCapital Community Care Pty Ltd will ensure that each participant who receive NDIS funded supports in relation to enteral nutrition and management has the right to quality of care to maintain and support their health and wellbeing. Whenever possible the participant must be involved with decision making and exercising choice about care and management of their specific health needs.
2.COMPLEX BOWEL CAREWe will ensure that any participant who requires support with complex bowel care receives an appropriate level of support that is relevant and proportionate to their needs. Any participant that receives support with complex bowel care is involved in assessment and development of their Complex Bowel Care Plan.
3.URINARY CATHETER MANAGEMENTAny participant that receives support with urinary catheter management will be involved in assessment and development of their management plan. The Plan and relevant procedure will identify how risks, incidents and emergencies will be managed, including required actions and escalation to ensure participant wellbeing
4. WOUND MANAGEMENTEach person who receive NDIS funded supports in relation to Complex Wound Management have the right to quality of care to maintain and support their health and wellbeing. Whenever possible the participant must be involved with decision making and exercising choice about care and management of their specific health needs. All workers working with a participant requiring complex wound management have training, relating specifically to the participant’s needs that are affected by their wound management regime (for example, showering, toileting and mobility) and high intensity support skills descriptor for providing complex wound management, delivered by an appropriately qualified health practitioner or person that meets the high intensity support skills descriptor for complex wound management.
5.Subcutaneous InjectionsWe will ensure that each participant requiring subcutaneous injections receives appropriate support relevant and proportionate to their individual needs and specific subcutaneous injections and medication administered.
Capital Community care can provide assistance with a various range of daily personal activities such as bathing and showering, toileting, grooming and medication assistance. Our experienced team of support workers will help you to achieve your goals.
Capital community care provides the assistance with domestic tasks including assistance with cleaning, dishwashing, clothes washing and other chores which will maintain a safe, secure and healthy environment for our clients.
Capital community care actively promotes and encourages our support workers to take part in what brings joy and happiness to your life. Our participants can take part in various community, social and recreational activities. It is also an opportunity to make new friends and get involved in the community because it can be difficult for many people living with disability. We are strongly believe that getting active involvement in community plays a vital role in overall wellbeing of the participants.
We can connect the participants with many innovative programmes to help them to get involved in their community. Each service is tailored to best suit the participant, and their goals to allow them to get the most out of the service. Our team will assist the participants to create a personalised programme for community participation and finding the employment.
It is important to maintain a sense of independence. Our support workers will either drive the participants or assist them to engage with the community. They will drive you to appointments, shopping or any other activity you may need to attend. We can also teach the participants how to independently use the local public transportation options, according to their best ability.
